How to create a QR code for a Google Form

  1. 1

    Open the Send menu

Open your form in Google Forms and click Send. If your version shows a Publish button instead, publish the form first; the share link is then under the link icon.

  2. 2

    Copy the link

Choose the link tab (the chain-link icon), tick “Shorten URL” for a cleaner code, and click Copy.

  3. 3

    Paste and download

Paste the link into the generator above, pick your colors and download a PNG or SVG.

  4. 4

    Test on a phone

Scan it with a phone that isn't signed in to your Google account, to check that people see the form and not a sign-in page.

Check these form settings before you print

Most “broken” form codes are really form settings. Check these first:

  • The form is accepting responses (see the Responses tab) and, in newer versions of Google Forms, is published.
  • Sign-in isn't required unless you need it. Options such as limiting people to one response make everyone sign in to a Google account, which slows people down on phones.
  • If the form is restricted to your school or organization, people outside it won't be able to open it.
  • Use the short forms.gle link. A shorter link makes a simpler code that scans more easily.

Edit your form without reprinting

A Google Form keeps the same link while you edit it, so you can add questions, fix typos or change the theme after printing and the code keeps working.

The link only changes if you use a new form, such as a copy for next term's sign-up sheet or next year's survey. If you want the same printed poster to point at a different form each time, a Pro dynamic code lets you switch it whenever you like and shows how many people scanned it.

Getting more responses

The code gets people to the form; the form has to do the rest. A few habits help:

  • Say what the form is and how long it takes, for example “Scan for our 2-minute feedback survey”.
  • Print the code at least 2 cm (0.8 in) wide, and bigger on posters scanned from a distance.
  • Keep forms short and easy to fill in on a phone, since that's where most people will answer.
  • Place codes where people have a minute to spare: waiting rooms, exits and event tables.

Frequently asked questions

How do I make a QR code for a Google Form?

Open the form, click Send, choose the link icon and copy the link, ticking “Shorten URL” for a cleaner code. Paste it into the generator above and download your code.

Is a Google Form QR code free?

Yes. It's a free static code with no sign-up, no watermark and no expiry.

Will the QR code still work if I edit the form?

Yes. Editing questions, settings or the theme doesn't change the form's link. Only a brand-new form or a copy gets a new link.

Why does my form ask people to sign in?

Settings such as limiting responses to one per person, or restricting the form to your organization, require a Google sign-in. Turn them off if you want anyone to answer without signing in.

Can I see how many people scanned the code?

Not with a free static code. Google Forms counts responses, and a Pro dynamic code also counts scans, so you can compare how many people opened the form with how many answered.

Should I use the long or the short form link?

The short forms.gle link. Both open the same form, but the shorter link makes a less dense code that scans more easily at small sizes.
